Call with the patient's name, diagnosis, insurance, phone number and whether the patient needs a PCP referral.
Important: Please provide needed documentation.
Documents required are medical insurance card , a valid state ID card and completed new patient packet.
Patient appointment cancellations should be done at least 24hrs. prior to appointment date. Otherwise the patient might risk not being accepted into practice. Referring physician will also be notified for missed appointments.
Please arrive at least 15 min. prior to appointment time in order to complete required documentation.
